Hi, I agree with the others that the compact UVB could very well be causing the loss of appetite. It would be a good idea to turn it off even for a wk and see if you notice any difference in him. In the meantime, order the Repti-Sun 10.0 fluorescent bulb.
As ABHD mentioned, antibiotics are very drying and can cause dehydration. Let us know how he acts when you give him a bath every other day.

As you know, 146 grams is a very low wt for an 11 mo old. Have you thought about trying other feeders besides superworms, perhaps he'd like silkworms, hornworms, butterworms or dubias. In the meantime, it would be a good idea to start feeding him some chicken and squash baby food diluted with a little bit of water, you can even add a pinch of calcium or vitamins to it, heat the mixture in a microwave for a few seconds to take the chill off and put drops on his nose with a plastic medicine dropper for him to lick off. Once he gets a taste of it, he should really like it. What greens have you been offering?
How often have you been dusting with calcium and vitamins?
Have you considered getting him a glass tank? Craigslist has some very reasonably priced. Perhaps your beardie would be more active which would increase his appetite if he had more of a view.
