I added a second light it went to 117 ...how high can it be for a baby ?

A 4 month old is getting out of the baby stage and becoming a juvenile, and they can't handle temps that high. That's even quite high for a baby. Try and get it 100-105*F. I just realized you were saying baby, but meant a 4 month old, so the 102*F temp is okay, ,but getting closer to 105*F is best.

AND can you please answer my question about the type of thermometer and how long you are leaving it in place? I really can't help you without information from you.
