Possible MBD?

My (almost) 4 year old bearded dragon has had muscle spasms and the end of his tail is twitching. This is the first time he has presented with symptoms like this.

He recently got treated for an URI by his vet and was given a clean bill of health last week. All throughout his antibiotic treatments he was twitching and it has worsened. We switch his diets from pellets (I know they are bad now) to collards, squash, etc. Since about April. He's been getting liquid calcium(daily) health slurries(daily) and his vegetables are being dusted with bee pollen daily and then alreating days for multivitamins, probitoics, and calcium powder. He eats regularly but is generally more lethargic, and has been basking and pooping less often as time progresses. Im not sure if it will just take times for the symptoms to stop or if it's not actually MBD.

His last 2 stools were runny but I'm not sure if it's from the Slurry and he usually brumates starting August. He is about to shed which he hasn't done in a while and idk if that's a sign that he's getting better or not.

He is in a 40 gallon tank with a 100w heat bulb and Reptisun 10 UVB bulb that was replaced in the end of March (and is regularly replaced every 6 months).
Temps 112f on the warm side and 88f on cool side.

Any help would be much appreciated!

Ok so I am taking its a T 5? If so is it in the tank or on top of the tank - I want to eliminate the UVB for cause of the MBD -- thats usually the first cause

Was there a X ray done on him ? And the 112 is too warm for basking usually older dragons do not like it that warm - do you offer a heat gradient where he can pick a basking spot and temp that he likes?

It's a T8 and its on the top of the tank which is a mesh cover. No X-ray was done on him and his branch had varying levels of incline. His heat bulb is a dimmer so i can play around with it until I get a better temp. Its warm and humid in the summer in NYC so I always struggle with his heating this time of year

I just set Swordtail's timer for his bath and paused it so I could actually fill his soaking bowl up and he crawled over my phone and canceled the timer 🤣
Mirage came out of brumation on April 26. He was doing great. On May 2 he started acting funny. We just redid his tank, and he keeps going into one of his hides. He just lays there. He shows no intrest in food. HELP!
