That isn't from the way he sleeps, most all dragons sleep with their arms folded in all kinds of crazy positions, underneath them, over top of them, folded in positions that you would think would hurt badly, and this doesn't happen. It is obviously injured, no doubt about that at all, it's just a matter of how badly it's injured and whether or not it's dislocated.
It looks like it could be dislocated, and the problem with that is that it will not ever heal on it's own if it is dislocated, it has to be reduced. The shoulder is a "ball-and-socket" joint, just like hip but less restricted than the hip and much easier to dislocate. Once the head of the humerus bone (the "ball") comes out of the "socket", their definitely can be a loss of blood supply, as all the blood vessels that supply blood to the arm/leg and the hand/foot and toes run right around the head of the humerus...However, the biggest issue with a dislocated shoulder being dislocated for a long period of time is that every single nerve that supplies sensation to the top of the leg/arm down to the tip of the toes runs through the "armpit" and will most always become compressed when the head of the humerus comes out of the socket, and will stay compressed until the shoulder joint is reduced and put back into place...this area of nerves in the armpit is called the "Brachial Plexus", and it consists of all 9 nerves that supply feeling to the entire leg, foot, toes, and top of the chest and neck (supplies all the sensation to the nerves that run through the pectoral muscles on that particular side and to the deltoid on that side)...
So while there is a chance that the blood supply to his entire leg, foot, and toes could be interrupted if it is dislocated, there is a much greater chance that the nerves in the Brachial Plexus are compressed and he will lose the ability to use that entire leg/foot. A simple, inexpensive, plain-film x-ray will show clearly whether or not the shoulder is dislocated, whether or not there are any fractures, and also whether or not there's been any bone death in the ball of the humerus due to reduced blood supply. It's definitely worth the at most $150 to have a simple, quick, non-invasive x-ray done ASAP, just to make sure that he's not permanently disabled, as well as he's not in tremendous pain, because if it is dislocated or there happens to be a fracture, Serrapeptase isn't going to do a thing to help that kind of pain, he would need a narcotic. It's already been a couple of days, I'd not wait much longer at all, the key here is if he continues to hold his leg like that, I'd say if he's still holding it like that tomorrow you need to get him to any vet ASAP for an x-ray, even a general vet can do the x-ray and diagnose a dislocation or a fracture, and if there is a problem they can refer him to a Reptile Specialist to reduce the dislocation or set the fracture, and they can also prescribe him the much needed narcotic pain med. Serrapeptase serves it purpose, it really does, but not for a dislocation or fracture, not at all.
Also, I'm assuming that your Reptisun 10.0 UVB tube is a T5HO strength, since you said it's a 22"? If it is the T5HO strength then you're fine to have it on top of the mesh lid. However, if it's the T8 strength you absolutely cannot let it sit on top of any mesh lid, and you must poke 4 holes in the mesh lid and then use either large plastic zip ties, wire, string, twine, shoelaces, etc. to strap the entire fixture to the underside of the mesh lid. This isn't optional with a T8 strength UVB tube of any brand, not Reptisun 10.0 or Arcadia, or any other brand, as the metal mesh lids block around 40% on average of the UVb light emitted by the tube, and the 15 or 17 watt T8 strength UVB tubes are no where near strong enough to still penetrate adequate UVB through to the other side of the mesh and to your dragon. As I said, due to the 22" remark I'm assuming it's the 22" Reptisun 10.0 T5HO UVb tube, which is 24 watts and is fine to sit on top of the mesh as long as he's within around 11" of the tube, 24 watts is strong enough to take the 40% UVB light blockage and still penetrate adequate UVB light to your dragon on the other side, but after many solarmeter tests not any of the 15 or 17 watt T8 tubes emit anywhere close to adequate UVB light for a dragon on the other side of the mesh lids. Many people still use the T8 UVB tubes for their Bearded Dragons, they just strap the entire tube fixture to the underside of the mesh lids and make sure that the Basking Spot/Platform is within at least 6" of the now unobstructed tube. It's easy, takes 10-15 minutes....
