Cleaning, with sand?

So I have a three year old rescue that has lived in sand her whole life, has been doing fine, and for the time being, i'm going to keep it that way, perhaps i'll change it one day but not anytime soon. My curiosity has led me to the question: When cleaning the tank, what do I do with the sand? :?:

Replace it all, this is the only way to be sure that bacteria isn't festering in your dragons home. You can take everything out and shop-vac out the sand.

replace it all this is what i did.

I just set Swordtail's timer for his bath and paused it so I could actually fill his soaking bowl up and he crawled over my phone and canceled the timer 🤣
Mirage came out of brumation on April 26. He was doing great. On May 2 he started acting funny. We just redid his tank, and he keeps going into one of his hides. He just lays there. He shows no intrest in food. HELP!
