Help with lighting

Reptisun 10.0 t5 HO or Arcadia 12% t5 at 12" from basking spot. Length for a 40 gal should be a 22" bulb and fixture. Usually you want to cover 1/2 of the enclosure, but they don't make a 18" bulb. Wattage for a Reptisun 10.0 22" is 24 watts
As for a basking bulb, a PAR38 halogen from a hardware/home improvement store works great. You need whatever wattage will get your basking spot to 100-105. I suggest a dimmable fixture as this allows you to fine-tune the temps without moving the basking spot around.

Yes, it will need a fixture. I would consider buying a fixture/light combo like this one http://www.lightyourreptiles.com/22-t-5-ho-fixture-high-output-with-arcadia-d3-12-ho-bulb/ The fixture and bulb are excellent and this will sit flush if you put it on top of the tank.
Yep. Those are nice too. They ship with a 5.0 bulb though so that will need to be replaced with either a 10.0 ReptiSun or an Arcadia 12% (either brand will fit).

Mirage came out of brumation on April 26. He was doing great. On May 2 he started acting funny. We just redid his tank, and he keeps going into one of his hides. He just lays there. He shows no intrest in food. HELP!
