Is the tank glass? You could try covering 3 sides with cardboard or paper. This will make him feel more secure and stop him seeing his reflection in the glass.
Sorry to hear you're still having problems with your little guy. It sounds like your temps are right, your lighting setup is good, and you're feeding him consistently. Maybe he's just messing with you Seriously though, I would have him checked out by a reptile vet to rule out neurological issues (like atadenovirus, although the symptoms don't match exactly) since everything else seems copacetic.
Also, are you feeding him greens as well as crickets? If you are, what kind? If you aren't, he will need around 20% of his diet to consist of greens (collard greens, mustard greens, squash, etc - another website has an awesome list of what to feed them daily, occasionally, as a treat, or never) and 80% crickets/protein in his younger years. That number eventually flips around as he gets older tho. Sorry if you mentioned it, I just scanned the post and didn't see if you are or not.
It could be neurological, maybe a birth defect possibly.
Do you know what the humidity is in his tank? If it is too low, that can also cause issues as well.
Is he getting enough calcium right now? Since you have the correct UVB light, he should be able to absorb his calcium properly by now I would think.
Is he eating anything at all right now, or very little?
What type of thermometer are you using to measure the temperatures with?
