How long has it looked like this? Has Nugget shed in that area recently or not? Initially you can try putting some raw unpasteurized honey or silver gel on it, to see if that clears it up. It could be just pigmentation changes or bacteria trapped underneath of the scales from a retained scale previously. If those don't appear to help then you can try some anti-fungal cream to put on it, just in case it could be the start of a fungal issue. Is there anything in the tank she could have scraped it on?
