Nose looks raw from peeling / rubbing

I noticed something going on with my bearded dragons nose / tip of mouth when I got home from work today. Not really sure what it is. Can anyone help me? His mouth has also been very slight open at time recently. Pictures below. Thanks in advanced

Looks like he's been rubbing it against something as you mentioned. Maybe the rock or the side of the tank? Does he get time outside of the tank? Perhaps he's trying to get out and run around a bit. The wound should heal pretty quickly if you can get him to stop agitating it.
